Please submit the form below to request a major change prior to orientation. This form must be submitted at least five days prior to your orientation.

Major changes can affect the availability of orientation dates and you may need to reschedule if you have already made a reservation. If you have not received confirmation of your major change within three business days of submitting this form, email orientation@niu.edu.

Theatre Arts-Design and Technology is a limited admission major with a portfolio required. Please contact Stephanie Sailer and/or Bethany Geiseman before submitting this form.
